The fracture of the humeral anatomical neck is a extremely rare injury especially when it complicates a closed reduction of anterior glenohumeral dislocation. We report the case of a 47-year-old man who presents a dislocation of the shoulder with an undisplaced fracture of the humeral anatomical neck which was initially undiagnosed associated with a fracture of the greater tuberosity initially treated in the emergency department by closed reduction and whose X-ray control revealed an iatrogenic complete displacement of the fracture of the humeral anatomical neck , the patient was operated on in emergency and underwent osteosynthesis of the various lesions with proximal locked plat through a deltopectoral approach . after a mean follow up of 15 months. The patient had occasional shoulder pain without functional limitation, and the fractures were consolidated without detectable necrosis of the humeral head.
